Waivers & Intake Forms

Before utilizing our services, new guests are required to fill out an intake form for each service.  Our Services Information & Acknowledgments and the General Release of Liability and Waiver can be completed online or when you are at the studio for your session(s).

If you have enrolled in a membership, the membership agreement must be signed before your first scheduled appointment. You can complete this at the studio or online.
